Kensington and Frankford are becoming Philadelphia’s hottest neighborhoods. Yet they are still home to vulnerable and poorly-served residents with multiple health, wellness, and medical needs.

Even worse, there are few local options for care and prevention services. What the community needs—according to the Jefferson College of Nursing (JCN)—is a nurse-led health and wellness center to provide primary care and services, including behavioral health, nutrition education, smoking/drug cessation programming, and more.

Sage worked with a team of Jefferson Nurses on a feasibility study to identify the best approach to serving the community. Through community meetings, neighborhood outreach, and meetings with public officials and potential partners, Sage helped to create and market a one-page concept paper that shared the vision for the center and its intended value and impact. We also created an action plan to raise visibility for the proposed center, solidify the message, and bring all potential partners, stakeholders (both internal and external), and community groups to the table. With great community support, JCN is now raising funds to develop the center.
